At Compassionate Home Care, we are excited to announce our new partnership with PocketRN. Eligible Medicare beneficiaries may qualify for respite services, caregiver support, and care coordination through Medicare's Guiding an Improved Dementia Experience (GUIDE) model.

By participating in the program, eligible families may receive $2,625 in annual respite care services—equivalent to 76 hours of in-home care.

With PocketRN, a participant in Medicare's GUIDE model, and the Compassionate Home Care team, we work together to help families remain safely at home with the support they deserve.
